Why do you wish to become an Overwatch Transhuman Arm Unit?: OTA are essential in creating roleplay, though they are rarely seen. I had always enjoyed roleplaying as one.

Give a detailed role-playing example with how you would type it out in the chat with /mes etc as an OTA unit:

OTA.00001 enters the slums with three other overwatch units, AR2 held towards the ground at waist level. They raise it upwards, their head scanning the surroundings in front of themselves. They stop in their tracks, holding up one of their hands and looking back towards the other super-soldiers. The Overwatch unit extends their finger to the furthest back soldier, speaking at them. "Stay towards the back and make sure nobody surprised us," he turns to the other two, "come with me, we're going to find them. Amputate anyone you see henceforth."

The two other OTA follow 00001 as the third stays back, guarding the entrance. They make their way further in the slums, before arriving to a bend - around it, a large open area.

OTA.00001 slows as they arrive at the corner, once again halting the others as their visor peeks around the corner. They see coverage as they begin to move forward - hiding behind it, as they motion the two other units to come along as well.

They come up behind 00001, hiding alongside the same cover as they wait for the next directive.

OTA.00001 hears the pitter-patter of footsteps as they wait behind cover, knowing instantly what it is. They peek over the small coverage provided, viewing over multiple rebels carrying small weapons. They reach for their radio, whispering in. "Multiple 647-E's, engaging." They motion at the OTA to their left, speaking to them. "Move around and get a vantage point. Yell out and grab their attention, that is when we will open fire."

The OTA takes a moment to circle around the rebels, where they had setteled - moving to the opposite side of where the other two units are. They yell out behind the cover they had chosen, the rebels looking back at the origin of the noise.

OTA.00001 takes the loud yell as a signal, whipping his rifle over the cover as soon as he hears it. He unloads in short bursts at the small group, along with the OTA at his side.

The small group of rebels is instantly gunned down before they even know what happened, short yelps of pain being heard before their demise was met.

OTA.00001 unhooks his radio, speaking into it. "Dispose of the bodies and return to NEXUS, they have been dealt with."

Explain why it is important that you behave correctly as an Overwatch Transhuman Arm unit: OTA are even more crucial than CCA in standards, being the role-roleplayers of the server.

What are your past experiences with Overwatch Transhuman Arm characters if any, give a detailed summary: I've led Overwatch on servers (cough hiraeth cough) and I've taken a few more roles at that.

Not much to say here. Your application wasn't extraordinary necessarily, but your RP examples met the minimum, and it doesn't hurt to have a few decent roleplayers with the OTA whitelist hanging around for when things get rough.
